The Ballot has been slightly altered for the 2003 Season to accommodate changes requested by the tabulator to encourage complete balloting.

  You may download a copy as a PDF and print it off on any printer (copies are fine).

Please use the following address when mailing WATCH ballots in for tabulation.  Your WATCH representative will provide you with pre-addressed stamped envelopes.  If you have poor handwriting, please include a copy of relevant pages from the playbill (cast list/tech list).

Judges are encouraged to keep a copy of all ballots and playbills (programs) they submit to prevent loss in the mail.
